Our client is a well-established provider of community services, dedicated to enhancing the quality of life for vulnerable individuals, including the elderly, people with disabilities, and their carers. With a strong focus on delivering exceptional care and adhering to compliance standards, the organization seeks to empower individuals and promote independence within their community.

We are seeking an experienced Community Care Coordinator to join our client's team for the duration of 1-3 months, with the potential for an ongoing position. In this role, you will be responsible for leading and coordinating the community care team and ensuring the delivery of high-quality, person-centered services that comply with National Disability Insurance Scheme (NDIS) standards. This role requires strong leadership, a deep understanding of vulnerable populations, and the ability to oversee service delivery models that cater to the diverse needs of individuals and their families.

The ideal candidate will have extensive experience in community care and team supervision, with a strong understanding of the needs of vulnerable populations.



As a Community Care Team Coordinator, you may be required to;

• Oversee staff working within the NDIS business framework.
• Ensure compliance with practice standards and guidelines.
• Coordinate residential services for participants receiving Supported Independent Living and Specialist Disability Accommodation.
• Develop and submit service quotes to the NDIA and other funding programs.
• Manage safe work practices, identifying and mitigating risks.
• Ensure compliance with NDIS Practice Standards and the NDIS Quality and Safeguarding Commission.

What we are looking for in a Community Care Team Coordinator;

To be successful in this role, there are a few things we are looking for:

• Extensive knowledge of the needs of vulnerable and elderly people, including those with disabilities and their careers.
• Demonstrated experience in supervising teams.
• Broad understanding of the community care sector and its operations.
• Tertiary qualifications in Health, Community Services, Aged Care, Disability, or a related discipline.
